Routine Home Care

Routine Home Care is provided wherever someone calls home, such as a private residence, apartment, assisted living facility, or nursing home. Routine home care is usually provided over the course of several weeks, months, or even longer depending on when the patient is first admitted to hospice and the duration of illness.

Hospice team members make regularly scheduled house calls to assess the patient and provide additional care or other services to address the unique needs and desires of each patient. Hospice staff are also available at all times on an on-call basis. We have a unique model of clinical responsiveness after business hours to provide the highest level of support 24-hours each day to everyone we serve.

Hospice Care in the Home

Patients with advanced, life-limiting illnesses are often able to remain in the comfort and security of their own homes while receiving hospice care. Directed by medical doctors, managed by registered nurses and coordinated by a care team, hospice patients and their families enjoy individualized care plans and scheduled in-home visits. They also benefit from medical equipment, medication supplies and other resources to ease the care burden and improve comfort.

Services provided by hospice include:

• Nurse Case manager to oversee clinical needs
• Home visits by the hospice Medical Director
• Bathing and personal care by a certified nursing assistant
• Regular visits from the Hospice Chaplain, Social Worker and Volunteer Coordinator
• Hospice-trained and certified volunteer support
• Family bereavement services
• Medication management & pharmacy pick-up as needed
• Medical equipment and supplies

Hospice Care in Nursing Homes, Assisted Living & Care Homes

Hospice services are in-addition to care provided by facility staff. Spring Valley Hospice works hand-in-hand with nursing facilities to provide extra care to hospice patients and their families. As part of this supplemental assistance, a registered nurse will make scheduled visits to the patient one or more times each week. Spring Valley Hospice provides an on-call nurse at night, including weekends, to support facility staff if there is a “change in condition”. As part of the routine home care hospice benefit, all medications and supplies related to the patient’s hospice care for symptom control and pain relief are readily supplied.

Services provided by hospice include:

• Medical Director visits

• Hospice-trained and certified volunteer support
• Family bereavement services

• Regular individual patient care plan meetings with facility staff

Hospice services are covered by Medicare or Medicaid, private insurance, private pay or through charitable care. This care is provided to all in need of our services regardless of age, sex, race, creed, marital status, sexual orientation, color, national origin, religion, citizenship status, veteran status, illness, handicap, disability or ability to pay.

Hospice does not mean giving up hope! It is possible that a patient will improve, resulting in discharge from hospice services. Or the patient or caregiver might decide to stop hospice care to seek aggressive, curative treatment. Hospice services can easily be resumed, if needed, at a later time.
